What We Do
Exceptional Rehab is a contract therapy provider offering physical, occupational and speech therapy services for skilled nursing centers throughout the United States. All of our partner facilities are equipped with a skilled nursing unit to allow for more focused rehabilitation.
Lead by dedicated rehab directors, our therapists take the time to work closely with residents and the interdisciplinary team to craft a plan of care and to evaluate achievement through functional outcome measurements. Each individual is evaluated on up to 75 metrics from the beginning of treatment through to discharge.

Our Services
At Exceptional Rehab, our therapists work directly with residents to achieve their rehab goals. Structured around a patient-focused care module, our interdisciplinary team team develops comprehensive, individualized care plans to maximize residents physical, social, and emotional independence, safely and quickly.
Physical Therapy
Our Physical Therapists work with residents in training for increased mobility, gait stability, posture and positioning. Exercise programs are implemented to increase muscle function, coordination and endurance. Individualized treatment may also include joint and soft tissue mobilization to increase range of motion, wound care and pain management. Fall Prevention and education is also provided
Speech Therapy
Our Speech-Language Pathologists address communication and swallowing dysfunction. Treatment plans can include recovery of speech, language and memory skills. Activities for memory, orientation and cognitive abilities may be implemented. Oral muscle strength and functioning required for speaking and swallowing, as well as appropriate/safe diet recommendations, may be designed for individual needs.
Occupational Therapy
Our Occupational Therapists work with residents in education and training in such areas as dressing, bathing, eating, grooming, and other activities of daily living. Adaptive techniques or equipment to overcome physical barriers may be recommended. Strength and coordination exercises are implemented to increase more independence in daily activities.
Outpatient Therapy
Some centers also offer outpatient therapy for area residents. Our licensed therapists provide the same quality level of care on an outpatient setting for individuals who have transitioned to home but still need ongoing therapeutic care in our outpatient therapy gym.
